Sample CSV file for investment transactions

Howdy.
I have a ton of old mutual fund purchases I would like to get into Quicken 13, but it looks like you can only enter them one at a time.  (for instance, I’ve bought HACAX monthly since 1995)  It would be easier to type it into Excel.  I heard your program can convert from Excel to QIF, but how do I know how to format the Excel file have the data in the right columns for brokerage entry?
